The most important thing that you need for an accessible design is to make the hallways and doorways wide enough. Another thing is to put blocking in the walls where you may have grab bars in the future.
Ramping makes the front porch accessible for wheelchairs, and then the threshold on a typical house is raised for water protection. On an accessible house, you would only want a half-inch threshold at the front door. The main issue is getting wide enough hallways and getting wide enough doors. That's something that's hard to retrofit, but the doorway is critical.
We have done several houses that are designed specifically to be accessible. We do that all the time in commercial; that's something that you would really need an architect for because if someone is not licensed, they don't keep up with handicap accessibility. We take a class nearly every year and keep up with all the codes for our commercial work.
